Jennifer Jerde, RN, BSN, Ridgeview Clinics, is the Lead RN Health Navigator for Ridgeview Medical Center’s primary care clinics located in the west metro. She has been influential in expanding the role of nursing in the ambulatory care setting while leading initiatives to improve patient outcomes.
Jerde holds a Bachelor of Science in Nursing from South Dakota State University in Brookings, South Dakota. Prior to joining Ridgeview in 2013, Jerde worked for the University of Minnesota Health Masonic Children’s Hospital in the pediatric bone marrow transplant unit.
In her role at Ridgeview Delano Clinic, Jerde became an advocate for care team collaboration and was instrumental in developing and operationalizing the Ridgeview Nurse Navigator program. This program is now implemented in all 12 of Ridgeview primary care clinics.
Jerde was also a key member of the Ridgeview team that implemented the Transitional Care Management process, an emergency and inpatient discharge follow-up program for patients, employers and the care system. Additionally, Jerde has helped Ridgeview Clinics implement standardized processes for asthma care and diabetes management, correlating directly with improved patient outcomes.
Jerde and her husband recently welcomed their second child in August, who joined big brother Jace.
